  • Log into the Skyline Desktop Connector

  • If the Desktop connector is hidden

    • Click on the ^ (hidden icons) in the bottom right system tray

    • Right click the Skyline icon

    • Click on show cues

  • Click on the Open Browser Application - Skyline will open in a web browser

  • Click on the Health Checks box or Health Checks in the top menu

  • Under cohorts - click on the + next to Health Checks

  • Click on Patients To Be Invited For a Health Check

  • Click on the patient - the line will be highlighted in white

  • Click on the Action dropdown in the top right of the screen

  • Click on Health Check - the assessment template will open

  • Populate the fields

  • Click the radio button to complete the Health Check

  • Tick the box to generate Patient Plan

  • Click on Finish

  • Heart Age Calculator - displays the results and allows you to show the patient how by changing their lifestyle they can lower the risk

  • Documents To Print - Patient Plan is available here. You can click Print against a single document or click Print All

  • Codes - the codes generated by the Health Check results will be listed - click Add Codes

To carry out a Health Check follow the steps below:

  • Login to Skyline via the desktop connector or directly onto the website

  • Launch the web interface via the open browser application option in the Desktop Connector (if the web page is not already loaded)

  • Select the Health Checks Solution Cohorts

  • Select the Patients to be invited Cohort

  • Find the patient to be reviewed either by scrolling though the patients or by entering the Name into the filters box.

  • Select the patient by clicking on them and then select Health Check from the Action menu

  • A new tab will open containing the Health Check review screen

  • Answer the questions - some data will be populated from the data in the patient record

    • Mandatory questions will be labelled

    • Values that are out of range will also generate an error message

    • As the questions are answered different options may appear

    • Calculators are indicated by a green square, click on the square to open the calculator if the result has not populated

      • When all the required values have been populated clicking on calculate will perform the calculation

      • The result of the calculation will be displayed

  • Once all the mandatory questions have been answered the finish button will be enabled

  • The box "Tick to generate patient plan" must be ticked to produce a patient plan

  • Clicking on Finish will produce the Heart Age Calculator, Documents to Print and Codes tabs

    • The Heart Age Calculator tab is populated with data entered during the Health Check but these values can be altered in the tab to show the effects of lifestyle change on the QRISK score. No data collected in the Health Check will be affected by changing the information in the Heart Age Calculator.

    • Any documents produced by the Health Check (e.g the patient plan) will be available to be printed on the Documents to Print tab

    • Once the documents have been printed click on the codes tab to review the codes that will be added to the patient record

    • Unticking the boxes against the code lines will stop that code being saved to the patient record

    • Clicking on Finish at the bottom of the screen will save the selected codes to the patient record and close the tab
