It is the CCG/PHD responsibility to manage the users granted access to CCG Skyline. Below are the forms needed to submit a Skyline User Request and a Skyline Deactivations Request for when staff leave. |
The CCG/PHD PHB can request a current list of active CCG/PHD PHB Skyline users by emailing Support@ishealth.co.uk and requesting this.
For ALL new CCG/PHD PHB Skyline users and deactivations, a form must be submitted.
To submit a CCG/PHD PHB Skyline User Request please click on the link below and select Super Organisation User. Note, we require the CCG/PHD PHB name and ODS code if known :-
