  • The patient is currently a registered patient of the practice

  • The patient is aged between 40 and 75 before the end of the Health Checks year

    • Note, while patients who are 75 during the Health Checks year will not be listed for invites, if a Health Check is carried out before the patient is 75, they will be counted as eligible.

  • The patient does NOT have any one of the following sets of codes on their clinical record:

    • Coronary heart disease

    • Stroke

    • TIA

    • Heart failure

    • Familial Hypercholesterolaemia

    • Peripheral arterial disease

    • Chronic kidney disease (CKD) stage 3, 4 or 5

    • Palliative care or terminal illness

    • A code indicating that an NHS Health Check is not appropriate

    • A code for cardiovascular disease (CVD) risk with a value (>=20) indicating that the patient is at high risk of coronary artery disease OR a code indicating that the patient is on the CVD high risk register

    • A diabetes code without a subsequent diabetes resolved code

    • A hypertension code without a subsequent resolved code

    • An atrial fibrillation (AF) code without a subsequent resolved code

    • A statin code in the 6 months before the Health Check or the 6 months before the start of the QOF year

    • A code indicating that they have had a Health Check in the four years before the start of the current QOF year.

Note, if a patient is on a Ace Inhibitor they will still be eligible for a Health Check unless they have a diagnosis, hypertension for example.
