The patient is currently a registered patient of the practice
The patient is aged between 40 and 75 before the end of the Health Checks year
NB Note, while patients who are 75 during the Health Checks year will not be listed for invites, if a Health Check is carried out before the patient is 75, they will be counted as eligible.
The patient does NOT have any one of the following sets of codes on their clinical record:
Coronary heart disease
Stroke
TIA
Heart failure
Familial Hypercholesterolaemia
Peripheral arterial disease
Chronic kidney disease (CKD) stage 3, 4 or 5
Palliative care or terminal illness
A code indicating that an NHS Health Check is not appropriate
A code for cardiovascular disease (CVD) risk with a value (>=20) indicating that the patient is at high risk of coronary artery disease OR a code indicating that the patient is on the CVD high risk register
A diabetes code without a subsequent diabetes resolved code
A hypertension code without a subsequent resolved code
An atrial fibrillation (AF) code without a subsequent resolved code
A statin code in the 6 months before the Health Check or the 6 months before the start of the QOF year
A code indicating that they have had a Health Check in the four years before the start of the current QOF year.
Note, if a patient is on a Ace Inhibitor they will still be eligible for a Health Check unless they have a diagnosis, hypertension for example.
