It is the CCGICB/PHD responsibility to manage the users granted access to CCG ICB/PHD Skyline. Below are the forms needed to submit a Skyline User Request and a Skyline Deactivations Request for when staff leave. |
The CCGICB/PHB PHD can request a current list of active CCGICB/PHB PHD Skyline users by emailing Support@ishealth.co.uk and requesting this.
For ALL new CCGICB/PHB PHD Skyline users and deactivations, a form must be submitted.
To submit a CCGICB/PHB PHD Skyline User Request please click on the link below and select Super Organisation User. Note, we require the CCGICB/PHB PHD name and ODS code if known :-
