As most Bury practices have to ‘part save’ the 1st Health Check as bloods are taken during the clinic, a cohort has been added to help complete the 2nd Health Check needed once the bloods are back and to complete any missed Health Checks in Skyline.
Bear in mind, this is ‘possible' incomplete Health Checks, we can't be 100% sure but if the patients listed they will be in the Eligible Patients cohort.
The Possible Incomplete Health Check cohort looks for patients that meet the following criteria:-
Eligible for a Health Check
No Health Check recorded this year
They have a cholesterol code in the last 6 months, and a BMI in the last 6 months, and Smoking status recorded in the last 6 months and Alcohol consumption recorded in the last 6 months.
