Skyline User Request & User Deactivation Forms

It is the practices / organisations responsibility to manage the users granted access to Skyline. Below are the forms needed to submit a Skyline User Request and a Skyline Deactivations Request for when staff leave.

For ALL new users and deactivations a form must be submitted.

The Practice Manager or administrator can request a current list of active Skyline users by emailing Support@ishealth.co.uk and providing your practices ODS / NACS code e.g. A12345.

You may want to check https://ishealth.atlassian.net/wiki/spaces/CSD1/pages/3079766022 document before submitting a Skyline User Request form; this will ensure the user is granted the right role for their job.

Note, only the Clinician role can complete Health Checks however the Clinical Support Assistant role can complete invites / recalls.
