What Are Health Checks

Overview

The NHS Health Check is a health check-up for adults in England aged 40 to 74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As patients age they have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

The Health Checks solution in Skyline is written to help Practices, Public Health Departments and CCGs manage the NHS Health Checks programme. The solution consists of three parts:

  • Cohorts (lists of patients)

  • Health Checks patient review data entry

  • Dashboards (Tables and Graphs)

Cohorts

The cohorts show lists of patients that are relevant to Health Checks. These can be used to identify patients who are eligible and who have had a Health Check.

This is a series of questions and calculators that show data from the patient record and prompt for additional information from the user. At the end of the Health Check data collected can be recorded into the patient’s medical record. There is also the ability to printout a personalised patient plan as well as any referral letters produced during the check.

Dashboards

Dashboards are available to the practice and also to the CCG or Public Health Department who is running the Health Checks programme locally. There are Dashboards to show aggregate figures for how many patients are eligible for a Health Check, how many patients have been invited for a Health Check, how many Health Checks have been completed. Other dashboards show numbers of patients who have been given advice, or referred and other outcomes that have been collected during a Health Checks review.
