Before Informatica support can check patients eligibility and the reason why not listed as Health Check Complete, however we would need to know their NHS number for investigationinvestigate the discrepancy, we need you to complete the checks below. If you still feel that the count is incorrect we would need example NHS numbers to investigate further.

Do The Patients Have A Health Check Completed SNOMED code On Their Notes

  • Are the different patients still listed in the Patient To Be Invited cohort?

    • YES - check

      • Check their clinical record for a Health Check Completed SNOMED code

    • NO -

      • Check if they’re in the Ineligible cohort (as above)

  • Are the different patients listed in the High Risk Review cohort (Bury practice only)?

    • YES - follow

    • NO - should

      • Should they be in the High Risk Review cohort?

        • YES - check they have a high risk register code and High Risk Review Done code on their notes - Bury High Risk Reviews
