The High Risk Review is no longer valid within Bury. The cohort will be listed until this can be removed in the next release of Skyline. Do not complete Health Checks on these patients.

The High Risk review cohort is something Bury implemented years ago though it has been suggested in the national documents. It certainly make sense to monitor High Risk patients.

How The High Risk Review Cohort Works

Within Skyline, Bury practices have a High Risk Review cohort.

If a Health Check was previously completed on a patient in iCAP or Skyline and their QRISK score was 20 >, a high risk register code is automatically added to the patients clinical system notes.

As long as there’s no diagnoses, the patient will be eligible for a High Risk Review.

The patient won't be eligible for a Health Check in 5 years time and therefore won’t be listed in the ‘Patients To Be Invited’ cohort but they will be in the ‘High Risk Review’ cohort where practices can complete the review should they choose to.

Running A High Risk Review

  • Select the patient in the High Risk Review cohort

  • Click on Action > Health Check

  • Complete the High Risk Review

Note, on completing the review you MUST tick both Health Check completed and High Risk Review done for the patient to update in the cohort.

Manually Adding The Health Checks Review Done Code In Emis

NOTE: If you’ve completed a High Risk Review and forgot tick the 'High Risk Review Done' box, this can manually be added this off the patients cue card.

NOTE: You need to add the High Risk Review Done code off the Skyline Desktop Connector

  • Ensure the patient is loaded in Emis Web

  • Desktop Connector has the patient loaded

    • Click on the + next to High Risk Review Due

    • Click on Actions under the Numerator Codes

    • Click on Save

Green ‘Data successfully Filed’ message is displayed

Health Checks Completed Outside Of iCAP Or Skyline - QRisk 20>

If a Health Check has been completed on a patient using the Emis template, and the patient has a risk of 20 >, they won’t automatically show in the High Risk Review cohort as a high risk register code will be missing.

If you want to use the High Risk Review cohort for monitoring your patients, they will need a high risk review code on their notes to put them in the cohort.

The high risk register codes are:-

417206009 : Cardiovascular disease monitoring
440358008 : Primary prevention of cardiovascular disease
161640005 : High risk of heart disease
