| Sharing data cross practices
| In testing we have identified a highly unlikely, but possible, condition where data might be shared inappropriately. For the condition to be a possibility, you need to meet both scenarios below:-
Note, if you meet just criteria 1, this is NOT an issue as you’ll be using your own Windows login. The use of generic logins is not compatible with Cyber Essentials or Data Security and Protection Toolkit accreditations so is considered highly unlikely to happen. |

| Unable to post Health Checks or ERICA data into Emis Web
| When click on SAVE an error is generated This has been been referred to Emis - certificate error. Workaround has been applied to Skyline so practices can continue adding data whist Emis and Informatica continue investigating |

| If there’s a Family History of CVD or Diabetes code on the patients record and it’s older than 2 months, the box will be displayed. If there’s no code for FH of CVD or Diabetes, the box is not being displayed. WORKAROUND Open the QRISK calculator and answer the FH of CVD question. For FH of Diabetes, the code would need to be added manually into the patients record. |

| The QRISK scores produced by a Health Check when the QRISK calculator is not opened does not consider a patient's postcode.
| The QRISK score is auto calculated once sufficient data has been entered. Unless the QRISK calculator is opened within the Health Check template, the postcode isn’t taking into consideration when calculating the score. Note, the lack of postcode in the calculation doesn’t aways impact the score. WORKAROUND To be sure that the postcode is always calculated within the score you need to open the QRISK calculator and click on Calculate before saving the Health Check SNOMED codes. Note, if the smoking status has also been changed during the Health Check, please reselect this in the QRISK calculator as per the workaround below |

| Newly added clinical data not being processed in Skyline as of the 12/6/2023. The impact is that any data added into your clinical system after the 12/6/2023 will not be reflected in Skyline. Note, this does not affect your clinical system data. Data added after the 12/6/2023 will not be prepopulated in the Health Check template or Risk calculator

| Health Check Template - email printing on the plan instead of the clinicians name
| Informatica are aware of an issue which only applies the first time your Skyline account is activated and a new password is set. The impact is the users email address prints at the end of the Health Check plan instead of the name. We’re already on it and have a workaround script fix which we’re running several times a day to correct the Skyline user name. Unfortunately there’s still a minimal risk that if you activate your Skyline account and complete a Health Check straight away, we may not have had time to run the script before you finish the Health Check. WORKAROUND Until the permanent fix has been deployed please check the end of the plan and ensure this has a name and not an email address. If your email address is printed on the plan, you may want to cross this out. |

| Health Check Template - manually ticking ‘Blood Pressure Treatment’ in the QRISK Calculator is adding the wrong code, all other manual edits work as expected
| We’re aware that most clinical staff don’t make changes to the QRISK calculator within the Health Check template as this is automatically calculated; however we want to make you aware of a bug we’ve identified IF you manually edit the QRISK score within the Health Check. During the Health Check, If you open the QRISK Calculator in the template, and ticking the symptom 'Blood Pressure Treatment', the code for 'Atrial fibrillation' is added to the coding tab in error. Please note, other symptoms are fine. WORKAROUND If you manually edit the QRISK Calculator and tick ‘Blood Pressure Treatment’, this will calculate the risk correctly BUT before adding the codes you MUST deselect Atrial fibrillation (49436004). You will then need to manually add Hypertension (38341003) into the patients clinical record. Note, this is not the case when using the Calculator from the Skyline Desktop Connector. |

| Health Check Completed tick box displayed when an alcohol unit of 0 auto populated
